Job summary

Avera Dialysis-Sioux Falls is seeking a Registered Nurse to coordinate care for patients with kidney disease across the care continuum. You will collaborate with nephrology, case management, and other departments to ensure timely, accessible care and optimal outcomes.

Responsibilities include assessing patient needs, documenting in the EMR, and guiding patients through the care plan while addressing barriers to care. A sign-on bonus may be available and PTO starts day one.

Qualifications

• Active RN license in the state of practice.
• Minimum of 2 years of dialysis experience.
• Excellent communication and teamwork skills.

Responsibilities

• Coordinates patient care across the dialysis continuum with multidisciplinary teams.
• Completes initial and ongoing needs assessments for patients.
• Documents assessments and referrals in the EMR and ensures follow-up.
• Educates patients and families and coordinates resources across departments.
