Job summary

Children's National Hospital is seeking a Clinical Pharmacy Liaison to support diabetes care by ensuring timely access to medications and supplies. You’ll collaborate with diabetes educators, providers, and care teams to optimize insulin, CGMs, and other therapies in a hospital setting.

The role requires knowledge of diabetes medications, strong organizational skills, and a patient-centered approach to deliver safe and effective care in a fast-paced environment.

Qualifications

• High School Diploma or GED required.
• 2 years relevant pharmacy work experience required.
• District of Columbia Pharmacy Technician License 30 Days required.

Responsibilities

• Coordinate Medication Ordering and Delivery: verify orders, procure meds, coordinate with local pharmacies, ensure timely delivery for inpatient and outpatient teams.
• Ensure Accurate Documentation: chart updates, inventory tracking, regulatory compliance.
• Partner with Local Pharmacies to Ensure Medication Availability: coordinate prescriptions and readiness for timely administration.
• Collaborate with Clinical Staff to Address Medication and Supply Requests: work with physicians, nurses, case managers.
• Support Inpatient and Outpatient Medication Management: manage orders and ensure post-discharge access to meds and supplies.
• Monitor Medication Therapy and Compliance: assist in adherence and address delays or concerns.
