Job summary

Endeavor Health is seeking a Diabetes Educator to join our on-site team at Naperville and Bolingbrook locations. The role involves educating patients on diabetes self-management, insulin use, and glucose monitoring while collaborating with a multidisciplinary team.

Ideal candidates hold a Bachelor’s Degree, an Illinois RN license, CPR certification, and CDE/CBDCE certification, with willingness to travel between sites and adapt to varying hours.

Qualifications

• Bachelor’s Degree required.
• Current State of Illinois RN license.
• Current CPR certification (AHA or Red Cross).
• Certified Diabetes Educator (CDE) / CBDCE certification.

Responsibilities

• Demonstrates knowledge to provide care appropriate to the age of patients served.
• Responds to physical and psychological needs of patients.
• Assesses patient condition based on observation, exam findings, tests, and physician’s diagnosis.
• Develops and implements a diabetes self-management plan for patients and families.
• Provides nutrition and insulin education and therapy plans.
• Supports ongoing plan updates as medical conditions change.
• Creates or sources patient education materials for primary duties.
• Educates on insulin administration and insulin pump programming.
• Counsels patients and families on prescribed therapy in an individualized manner.
• Participates in hospital and department quality improvement activities.
• Contributes to diabetes education training for staff and students.
• Keeps up to date with diabetes care trends through literature and CEU activities.

Education

Bachelor’s Degree

Illinois RN license
