Job summary

Johns Hopkins Community Physicians (JHCP) is recruiting a Diabetes Educator Certified (Dietitian) to support the Central Nursing diabetes education program. The role involves assessing and educating patients and families in an outpatient environment, bridging inpatient to outpatient care, and handling sick day guidance while coordinating with a multidisciplinary team.

Qualifications

• Bachelor’s degree in Dietetics.
• Current license or registration in a specific allied health profession (e.g. Registered Dietitian) required. Must obtain Certified Diabetes Care and Education Specialist (CDCES) within 1 year of employment.
• Knowledge of medical terminology, diagnosis, and basic disease processes. Analytical ability to gather data, decide on conformity based on predetermined criteria, identify problems and refer for resolution. Basic knowledge of medical records coding standards. Awareness of licensing and accreditation standards. Knowledge of MS PowerPoint, Excel, and Outlook. Knowledge of relational EMR data documentation, ability to conduct research of issue at hand, as well as formulate recommendations based on findings.
• Requires a minimum of 2 years’ experience.

Responsibilities

• Assess, plan, implement, and evaluate components of the diabetes education program in an outpatient setting.
• Provide diabetes education to patients and their families and serve as the diabetes nutritionist for patient education.
• Plan and conduct continuing education programs in diabetes and nutrition for allied professionals and the general public.
• Respond to phone calls from parents of newly diagnosed children and adolescents with diabetes; assist with transition from inpatient to outpatient care; address sick day management and day-to-day diabetes concerns.
