Johns Hopkins Community Physicians (JHCP) seeks a Diabetes Educator Certified (Dietitian) for Central Nursing, delivering diabetes education in an outpatient setting and coordinating care with families. The role supports transition from inpatient to outpatient care and requires CDCES within 1 year.

Candidates should hold a Bachelor’s in Dietetics, RD licensure, and possess strong knowledge of medical terminology, MS PowerPoint, Excel, and EMR documentation.

Qualifications

• Bachelor’s degree in Dietetics required.
• Current license or registration in allied health (e.g. Registered Dietitian) required; CDCES within 1 year.
• Knowledge of medical terminology, diagnoses, and basic disease processes.
• Minimum of 2 years’ experience.

Responsibilities

• Assess, plan, implement, and evaluate diabetes education program at the diabetes and endocrine practice.
• Provide diabetes education to patients and families in outpatient setting.
• Respond to phone calls from parents of newly diagnosed children and adolescents with diabetes.
• Address sick day management and day-to-day type 1 and type 2 diabetes concerns.
