• BLS healthcare provider certification.

Responsibilities

• Provide diabetes self-management education and MNT to individuals and groups.
• Assess educational needs and tailor nutrition plans accordingly.
• Coordinate patient care with physicians, pharmacists, case managers, and social workers.
• Educate patients and families and document care in EMR.
