Sky Lakes Medical Center in Oregon is seeking a Dietitian Diabetes Educator (FT Days) to provide comprehensive diabetes education and self-management training for outpatient populations. You will assess patients, develop meal plans, and document care in the EMR, collaborating with the healthcare team.

The ideal candidate is a registered dietitian or RN with CDCES eligibility and Oregon licensure, with strong communication and public speaking skills to educate patients and the community.

Qualifications

• Computer literacy and ability to assess patients.
• Teach Diabetes Self-Management and document in EMR.
• Preferred: 2 years diabetes education and CDCES within 2 years.
• Licensure as RD/RDN or RN; Oregon licensure required.

Responsabilités

• Provide comprehensive diabetes education and intervention for outpatient patients.
• Document in written and electronic medical records per policy.
• Develop and re-evaluate treatment plans with the healthcare team.
• Present community education and assist with department quality improvement.
• Serve as clinical resource to staff and stay current with diabetes practice.
• Coordinate care with physicians, nurses, and care teams.
