Job summary

Texas Children's Hospital seeks a Diabetes Educator to provide assessment, goal setting, education and behavioral intervention for patients and caregivers in both inpatient and ambulatory settings. You will promote diabetes self-management and coordinate care across teams in a fast-paced environment.

The role offers a clinical progression ladder, professional growth, and collaboration with multidisciplinary staff; candidates with RN, RD/RDN credentials or working toward CDCES/BC-ADM are welcomed.

Qualifications

• Bachelor's degree required; master's preferred.
• CDCES or BC-ADM certification required.
• 1 year of specialized diabetes care experience as licensed dietitian or nurse.

Responsibilities

• Teaches diabetes self-management skills to patient/family.
• Assesses knowledge and tailors education to learner needs.
• Evaluates needs and suggests management strategies, including diabetes technologies.
• Documents all care activities in the medical record.
• Develops education materials for staff, patients, caregivers, and community.
• Maintains current practice through continuing education.
• Implements protocols for diabetes patient management.
• Coordinates case management with other healthcare providers.
