Job summary

Cincinnati Children's Hospital Medical Center is seeking a Diabetes Care Educator for the Burnet Campus in Amb Svc Endocrinology. You will work with a multidisciplinary pediatric diabetes team, provide education to patients, families, and school staff, and coordinate disease management across outpatient services.

The role is full-time (36 hours weekly, 0.9 FTE) with a day shift. Requirements include a Bachelor's degree, OH Dietician or RN license, 1+ year of related experience, and CDCES

Qualifications

• Bachelor's degree in a related field.
• 1+ year of work experience in a related job discipline.
• OH Registered Dietician license OR OH Registered Nurse license.
• Must obtain CDCES Certification through the Certification Board for Diabetes Care and Education within 3 years.

Responsibilities

• Care Integration- Works as an effective member of a multi-disciplinary pediatric diabetes care team, and working with children with diabetes. Provide education to school nurses, school personnel and other care providers.
• Patient and Family Education- Provides pre-diabetes and diabetes instruction in group and individual sessions. Establishes individualized learning goals and educational plan.
• Outreach- Participates in community diabetes programs/outreach activities and supports quality standards.
• Clinical Practice- Assess DSME/S of patient and family and provide diabetes self-management education in outpatient settings.
• Program Support- Supports regulatory compliance efforts and contributes to quality improvement activities.
