Job summary

UW Health in Madison, WI seeks a Registered Nurse Educator to lead diabetes education initiatives and support patient care across settings. The role requires a Bachelor of Science in Nursing and RN licensure in Wisconsin, with at least 3 years of RN experience and experience in diabetes education.

You will provide education to individuals and groups, collect data to guide care plans, and collaborate with teams to meet patient goals.

Qualifications

Bachelor's Degree in Nursing is required.
Minimum 3 years of RN experience is required.
CBDCE certification as CDCES required within 2 years of hire.
BC-ADM certification within 2 years of hire.
RN licensure in the state of Wisconsin (or eligible for multistate licensure).
CPR/BLS certification required within 2 years of hire.
Experience as a Diabetes Educator is preferred.

Responsibilities

Provide education to individuals and groups with emphasis on diabetes education.
Systematically collect and assess data related to patient health to develop, implement and evaluate care plans.
Establish goals and strategies for meeting care coordination needs across settings with patients, families, and care providers.
