Qualifications

• Successful completion of a Registered Nurse education program.

Responsibilities

• Develops, maintains, and provides individualized diabetes self-education and care plans for participants; tracks quarterly progress.
• Collaborates with providers and other healthcare professionals on education plans and outcomes.
• Provides preventive education to the community and students.
