Job summary

Allegheny Health Network, part of Highmark Health, seeks a Diabetes Education Specialist to assess patient needs and provide individualized self-management education. The role will also plan continuing education programs for professional staff and the community.

Responsibilities include data collection, goal setting, follow-up planning, outcome evaluation, and documentation in participants’ records, with involvement in curricula development.

Qualifications

• Bachelor’s Degree Registered/Licensed Dietitian.
• Experience in counseling patients with diabetes and experience in community education.
• Preferred Master’s Degree.
• 1-3 years’ experience in Diabetes Education / Health Living Coaching.
• Certified insulin pump trainer (CPT).

Responsibilities

• Collects DSME/T program participant assessment data, using a recognized certification database/program in a collaborative and ongoing manner. (20%)
• Develops educational goals, learning objectives and a plan for educational content and teaching methods with program participants. (20%)
• Collaboratively develops an individualized follow-up plan with each program participant.
• Evaluates effectiveness of educational services provided by measuring attainment of learning objectives. (20%)
• Conducts a follow-up assessment upon completion of DSME/T program services, using outcome measures from the AADE7 Continuum of Outcomes Framework. (20%)
• Documents assessment data, educational plan, educational services provided and evaluation results in each participant's educational records. (10%)
• Participates in the development of education curricula and materials. (10%)
• Performs other duties as assigned or required.
