Job summary

Qualifications

• RN diploma or BSN required; master’s preferred.

Responsibilities

• Guide patients with diabetes to establish behavioral and clinical goals and develop education plans with learning strategies.
• Coordinate with the health care team to assess and revise patient care plans for satisfaction, safety and outcomes.
• Instruct individuals and families on diabetes education, disease management and self-care practices.
