Job summary

Ochsner Health in New Orleans seeks a Diabetes Care Education Specialist to partner with patients and the healthcare team to educate and support diabetes self-management.

You will assess patient needs, develop education plans, and coordinate with the care team to improve outcomes. Requires RN licensure and experience with diabetes education; Master’s preferred and CDCES encouraged. Strong communication and teaching skills a must; some evening/weekend hours may be required.

Qualifications

• RN licensure required or Bachelor’s degree in a related field.
• Master’s degree in a related field preferred.
• 3 years of direct patient contact experience required.
• CDCES certification preferred by CBDCES.

Responsibilities

• Guides persons with diabetes in establishing individualized behavioral and clinical goals and develops learning plans.
• Develops an education plan addressing gaps in knowledge and collaborates with patients to plan strategies.
• Consults with health care team to assess, plan, implement and evaluate patient care plans.
• Instructs individuals, families and groups on diabetes education and management topics.
• Recommends treatment options and devices as needed.
• Adapts communication to diverse patient populations and settings.
• Performs other related duties as assigned.
