Job summary

The Ohio State University Physicians, Inc. (OSUP) is seeking a Diabetes Care Education Specialist to deliver diabetes self-care education and coordinate patient education within the Division of Endocrinology, Diabetes and Metabolism.

You will educate patients, assess needs, and collaborate with referring providers to support optimal diabetes management. The role requires a Bachelor's Degree with OH licensure in a relevant field (RD/RN/Pharmacist) and a minimum of 2 years in diabetes care or

Qualifications

• Bachelor's degree required.
• Licensure in Ohio as RD, RN, or Pharmacist.
• At least 2 years of work experience in a diabetes specialty and/or patient education.

Responsibilities

• Conducts comprehensive diabetes assessments and develops care plans.
• Provides personalized diabetes education to patients.
• Coordinates care with referring providers.
• Develops Diabetes Self-Management Education (DSME) through books, PowerPoint presentations and patient education handouts.
• Provides nutritional and physical activity guidance; under physician supervision provides education and guidance for patients on medications, SMBG and CGM and diabetes technologies including insulin delivery device including insulin pumps, InPen and patch insulin devices.
• Serves as diabetes expert to the medical and hospital staff.
• Serves as integral member of the ADA Recognition education committee.
