Job summary

Ohio State University Physicians seeks a Diabetes Care and Education Specialist to deliver diabetes self-management education and coordinate patient care within the Division of Endocrinology, Diabetes and Metabolism of the Department of Internal Medicine. You will assess patients, create care plans, and provide education under physician supervision.

The role requires a Bachelor's degree and Ohio licensure (RD/RN/Pharmacist) with 2+ years in diabetes education; certification is a plus.

Qualifications

• Bachelor's degree required.
• Current licensure in the State of Ohio as RD, RN, or Pharmacist.
• At least 2 years of work experience in diabetes care or patient education.
• Certified Diabetes Care and Education certificate preferred.

Responsibilities

• Conduct comprehensive diabetes assessments and develop care plans.
• Provide personalized diabetes education to patients.
• Coordinate care with referring providers.
• Develop DSME materials and education handouts.
• Provide nutritional and physical activity guidance and insulin education.
• Serve as diabetes expert to medical and hospital staff.
• Participate in ADA recognition education committee.
