Qualifications

• Must be a graduate from an accredited school of nursing - or - must have a bachelor's degree in food and nutrition.
• Three years of clinical diabetes care experience required.
• BLS for Healthcare Providers certification required; must obtain CDCES or CDE within six months of hire.

Responsibilities

• Provide individualized diabetes self-management education and support per the scope of practice and standards.
• Perform comprehensive patient assessments and document education, plans, and outcomes.
• Collaborate with the multidisciplinary diabetes care team to develop goals and teach methods.
• Liaise with clinical and community partners to optimize patient self-management.
• Advocate for patients and ensure culturally responsive care; participate in diabetes education programs.
