R&M Welty INC. in Otsego, MN is seeking a Designated Coordinator to lead and supervise a direct care team in a full‑time daytime role.

The position emphasizes person-centered care, compliance with Minnesota 245D, and a focus on safety and quality of life for persons served. The role requires prior management experience in health/human services, strong communication skills, and the ability to train staff while ensuring documentation and technology use are up to date.

Qualifications

• Bachelor's degree or equivalent with direct care experience as listed in the requirements.
• Two years of direct care experience for an associate degree, or three years with a diploma.
• 50 hours of education and four years of direct care under supervision per MN 245D guidelines.
• Two+ years of direct staff management in health/human services settings.
• Experience meeting Minnesota 245D Designated Coordinator qualifications and ability to lead teams.

Responsibilities

• Lead, train, and motivate a diverse team while upholding person-centered and professional standards.
• Supervise daily operations and ensure compliance with Minnesota Statute 245D for assigned sites.
• Promote and protect health and safety of persons served per Minnesota Statute 245D.
• Provide training and guidance for Direct Support Professionals following applicable laws and protections.
• Guide teams to interact effectively with persons served, staff, guardians, and families.

What You'll Do:

• Lead, train, and motivate a diverse team while upholding person-centered and professional standards


• Supervise daily operations and compliance in accordance with Minnesota Statute 245D for assigned program sites


• Promote and protect the health and safety of persons served in accordance with Minnesota Statute 245D


• Provide training, support, and guidance for Direct Support Professionals following Minnesota Statute 245D and all applicable employment laws and vulnerable adult protections


• Guide teams to effectively interact with and among the persons served, staff, guardians, case managers, administrative team, community, and families of persons served

What You'll Need:

• Must meet one of the following educational requirements:
  
  
  
  • A baccalaureate degree in a field related to human services, education, or health and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children OR
  
  
  • An associate degree in a field related to human services, education, or health and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children OR
  
  
  • A diploma in a field related to human services, education, or health from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older or equivalent work experience providing care or education to vulnerable adults or children OR
  
  
  • A minimum of 50 hours of education and training related to human services and disabilities AND four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3) of the MN statutes for 245D
  
  
  
  


• 2+ years of direct staff management in a health/human services setting (required)


• At least one year of full-time experience in direct care and 50 hours of relevant training (required)


• Experience with Minnesota 245D Designated Coordinator qualifications


• Ability to lead, train, and motivate a diverse team while upholding person-centered and professional standards


• Proficient with technology and documentation


• Strong communication, organization, and time management skills


• Must be 21 years or older


• High school diploma or GED


• Valid driver's license and reliable transportation
