Empath Health is seeking a Dementia Services Care Navigator to perform psychosocial assessments for people living with dementia and their caregivers as part of the Nancy Renyhart Dementia Education Program (NRDEP).

The role involves outreach to professionals and communities, developing referrals, facilitating Virtual Dementia Tour trainings and caregiver support groups, and applying knowledge of Medicare/Medicaid related to skilled nursing facilities.

Formación

• Master’s Degree in Social Work with at least 1 year casework experience.
• Experience in counseling, crisis management, and grief support preferred.
• Knowledge of dementia care and community resources is helpful.

Responsabilidades

• Outreach to professionals and communities about NRDEP and dementia services.
• Assist people with dementia and caregivers to resume life in the community.
• Develop referral relationships with physicians and clinics.
• Facilitate Virtual Dementia Tour trainings as needed.
• Facilitate dementia caregiver support groups and ensure positive outcomes.
• Maintain knowledge of Medicare/Medicaid related to skilled nursing facilities.
