Job summary

Augusta University is seeking a Community Service Educator for the Memory Assessment Center to educate patients with dementia and their caregivers, and to connect them with resources and support services.

You will assess needs, coordinate care planning, and work with clinical and community partners to improve access to services. This role emphasizes compassionate education, referral optimization, and documentation integrity.

Qualifications

• Master’s degree in Social Work, Counseling, or Psychology is required.
• Or a Bachelor’s degree in Social Work, Counseling, or Psychology with a minimum of four years of community outreach/related experience.

Responsibilities

• Administer and interpret standardized needs assessments and integrate results into care plans.
• Establish and maintain linkages with resource personnel in the region.
• Assess patient eligibility for state programs such as SOURCE and refer appropriately.
• Maintain current clinical documentation per Memory Assessment Center requirements.
• Evaluate patient outcomes and adherence to care plans.
• Provide patient and family education tailored to needs.
• Collaborate with other professionals.
• Attend trainings and meetings as required.
• Perform related functions as assigned.
