Job summary

Qualifications

• Master’s degree in Social Work, Counseling, or Psychology is required.
• Or a Bachelor’s degree in Social Work, Counseling, or Psychology with a minimum of four years of community outreach/related experience.

Responsibilities

• Administer and interpret standardized needs assessments and integrate results into care plans.
• Establish and maintain linkages with resource personnel in the region.
• Assess patient eligibility for state programs such as SOURCE and refer appropriately.

Job Summary

The Community Service Educator will educate persons diagnosed with dementia and their caregivers, facilitate access to available public and private resources, and provide support services including counseling and care planning.

Responsibilities

• Effectively administer and interpret standardized patient and caregiver needs assessment tools and integrate the results into an overall plan of care.
• Establish and maintain linkages with key resource personnel in the region.
• Assess patient eligibility for state programs such as SOURCE and make effective referrals as appropriate.
• Maintain current and timely clinical documentation as required by the Memory Assessment Center.
• Evaluate patient outcomes and adherence to care plans.
• Provide patient and family education tailored to specific needs.
• Work collaboratively with other professionals.
• Attend and participate in trainings and meetings as required.
• Perform other related functions as assigned.

Required Qualifications

• Master’s degree in Social Work, Counseling, or Psychology from an accredited institution.
• OR a Bachelor’s degree in Social Work, Counseling, or Psychology with a minimum of four years of community outreach, community linkage, or related experience.

Preferred Qualifications

• Degree in Social Work and/or Psychology.
• Experience working and conducting assessments with persons living with dementia and their caregivers.
• Experience conducting assessments with older adults.
• Experience in healthcare and geriatrics.
• Clinical experience: 4+ years providing care consultation and case management.
• Demonstrated experience in quality program delivery and evaluation.

Knowledge, Skills, & Abilities

Knowledge

• Mental health issues common among persons with dementia and their caregivers.
• Non-pharmacological interventions for managing dementia-related behaviors and common mental health disorders.
• Interventions to reduce caregiver burden and improve caregiver physical and mental health.
• Clinic workflows and scope of practice for primary care.
• General resources available to incorporate into care plans.

• Develop and maintain effective working relationships with physicians, staff, patients, families, and agency staff.

Shift / Salary / Benefits

Shift: Days, Monday‑Friday (outside normal business hours may be required for an exempt position).
Pay Band: B11
Salary Range: $56,600 to $72,200 annually (commensurate with qualifications).
Benefits may include health, dental, life insurance, Teacher Retirement System or Optional Retirement Plan, earned vacation, sick leave, and 13 paid holidays.
