Qualifications

• Current LPN licensure in Ohio is required.

Responsibilities

• Provide direct patient care, including administering medications, wound care, and treatments as prescribed by physicians.
• Conduct vital signs, monitor resident conditions, and report changes to the healthcare team.
• Develop and implement individualized care plans, in conjunction with the interdisciplinary team.
• Collaborate with rehabilitation staff to develop and implement treatment plans.
• Participate in quality improvement initiatives to ensure optimal resident outcomes.
• Maintain accurate and timely documentation of resident care and services.
• Assist with admissions, transfers, and discharges as needed.
• Perform other duties as assigned by the RN or Nurse Manager.
