Job summary

Advantage Home Health seeks a compassionate Occupational Therapist to join our Home Health team on a PRN basis in Franklin County, PA. You will work in patients' homes, managing your schedule to provide one-on-one care and help individuals regain independence in meaningful activities.

Responsibilities include evaluating and treating, educating patients and caregivers, and collaborating with the care team to achieve optimal outcomes. A PA OT license, CPR, and a valid driver's license are required.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Occupational Therapist license in Pennsylvania.
• Current CPR certification (or ability to obtain upon hire).
• Valid driver's license, reliable transportation, and proof of auto insurance.
• Strong communication, organizational, and documentation skills.
• Home health experience preferred, but not required.

Responsibilities

• Provide skilled occupational therapy services in accordance with physician orders and individualized plans of care.
• Evaluate patients and develop personalized treatment programs to improve function, safety, and independence.
• Assess patients' ability to perform activities of daily living (ADLs) and recommend adaptive techniques or equipment as needed.
• Document patient evaluations, treatments, and progress in a timely and accurate manner.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure optimal patient outcomes.
• Educate patients and caregivers on home safety, adaptive strategies, and ongoing therapy goals.
• Supervise Certified Occupational Therapy Assistants (COTAs) as appropriate.
