Job summary

Partnersforcommunity is seeking a Day Support Specialist to engage individuals in community-based activities and skill-building under a person-centered approach.

The role requires a high school diploma or GED, a valid driver’s license, and certifications within six months of hire. The position offers flexibility with full-time and part-time schedules, and opportunities to help participants grow and participate meaningfully in their communities.

Qualifications

• High school diploma or GED required.
• Valid driver’s license and reliable transportation required.
• Knowledge of Connecticut DDS systems is preferred.
• DDS Medication Certification, First Aid, CPR, and PMT certifications must be obtained within six months of hire.
• Experience supporting individuals with intellectual or developmental disabilities preferred.
• Strong communication, organizational, and documentation skills.
• Ability to work collaboratively with patience and professionalism.

Responsibilities

• Actively engage individuals in meaningful activities within program and community settings.
• Teach, assist, and supervise individuals during activities, employment opportunities, and skill-building exercises.
• Plan community outings and provide individuals with multiple activity options.
• Assist with personal care while maintaining dignity, privacy, and safety.
• Support individuals in achieving goals identified in their ISP.
• Document ISP objectives, attendance, behavioral data, participation, and progress.
• Maintain organized and current case records.
• Report changes to supervisor or team.
• Safely transport individuals between locations and maintain vehicle logs.
• Report vehicle damage or safety concerns promptly.
• Perform additional duties assigned by the Program Coordinator.
