Job summary

Guardian Angels Care Center in Elk River, MN seeks a Transitional Care unit assistant to accurately administer medications and treatments under licensed nurse supervision, and to notify nurses of changes in condition. The position requires a Trained Medication Assistant program and MN Nursing Assistant registry, with long-term care experience preferred.

Qualifications

• Must have completed Trained Medication Assistant program.
• Currently registered as a Nursing Assistant with the Minnesota Nursing Assistant registry.
• Experience in long-term care or geriatrics preferred.
• Excellent verbal and written communication skills.

Responsibilities

• Accurately administer medications and treatments as ordered by the physician or designee and under the supervision of a licensed nurse.
• Promptly notify nurse of falls, bruises, skin tears, and change in condition.
• Monitor assigned nursing assistant to ensure compliance with resident plan of care and facility policy and procedures.
• Provide report to nursing assistants.
• Accurately documents cares provided and resident observations.
• Provide a dignified, resident-centered environment for residents.
• Follow universal precautions, infection control guidelines, and safety standards of practice.
• Uphold HIPAA and protect confidential information.
• Perform all other duties as assigned.
