Trinity Health Hospital in Grand Rapids, MI, is hiring a full-time RN Case Manager (32 hours/week, day shift). The role coordinates discharge planning and utilization management, guiding patients and families through the health care continuum to ensure high-quality, cost-effective care.

The position requires a BSN or ADN with BSN pursued within 4 years and an active MI RN license; preferred: Case Management certification and 2+ years of acute care CM experience.

Qualifications

• BSN minimum; or ADN with BSN to be obtained within 4 years of hire.
• Current RN license in Michigan.
• Preferred: Certification in Case Management or related certification.
• At least 2 years of Case Management experience in an acute care hospital.
• Preferred: 5+ years of Case Management experience.

Responsabilités

• Provide a competent patient assessment and interpret data to inform decisions and plans.
• Assess patient needs and plan for discharge with evidence-based practices and care coordination.
• Manage cases per organizational standards and ensure continuity of services across the care continuum.
• Coordinate care plans with health care team, patient, and family/support systems.
• Engage in active listening and empathetic communication with patients and families.
