As an LPN/RN, you will work with a team of healthcare professionals to assess, plan, implement, and evaluate patient care, administer medications, monitor vital signs, and assist during examinations. Current LPN/RN license in the state is required, with at least one year of long-term care

Qualifications

• Current LPN/RN license in the state.
• At least one year of LPN/RN experience in a long-term care setting is preferred.
• Knowledge of MatrixCare preferred.

Responsibilities

• Consults and coordinates with healthcare team members to assess, plan, implement, and evaluate patient care plans.
• Prepares and administers medications and records prescribed treatments per policy.
• Educates patients on procedures and records vital signs and medical information.
• Monitors and adjusts equipment and interprets electronic displays.
• Initiates corrective action for adverse symptoms and participates in discharge planning.
• Performs bedside care for a wide variety of medical patients, including pre- and post-surgery.
