Job summary

Wellpath is seeking a Licensed Practical Nurse (LPN) in Kentucky to join our care team. The role supports patient care under RN supervision, administering medications and IV therapy where certified, and contributing to treatment plans with a focus on compassionate, high-quality care.

Required is graduation from an accredited nursing program, active LPN/LVN licensure in the state, and CPR certification; a minimum of one year of clinic experience is preferred.

Qualifications

• Graduate from an accredited school of nursing.
• Minimum of 1 year clinic experience preferred.
• Current LPN/LVN licensure in the state of employment.
• Must maintain CPR certification.

Responsibilities

• Under RN supervision, assess patients’ physical, psychological, and social dimensions.
• Administer medications and IV therapy as certified.
• Assist in coordination of care and patient education.
• Document nursing encounters using SOAP charting and ensure effective communication.
