Job summary

Heritage Grove Estates in Topeka, KS is seeking a dedicated LPN Day Shift to provide direct nursing care, administer medications, and monitor resident health within a skilled nursing setting. You will collaborate with RNs, CNAs, and other care professionals to ensure aligned care plans, timely documentation in the EMR, and regulatory compliance while supporting resident and family education.

Qualifications

• Graduate of an accredited LPN program with an active Kansas license.
• Current BLS or CPR certification.
• Ability to provide resident-centered care across varying acuity levels.
• Proven ability to collaborate within a multidisciplinary healthcare team.
• Strong clinical judgment and communication skills.
• Commitment to quality care and regulatory compliance.

Responsibilities

• Deliver direct nursing care per individualized care plans and orders.
• Assess resident conditions and report significant changes to clinical staff.
• Administer medications and treatments per policy and scope of practice.
• Collaborate with interdisciplinary team to ensure continuity of care.
• Educate residents and families about diagnoses, medications, and care plans.
• Supervise and delegate tasks to CNAs and support staff.
• Maintain timely, accurate EMR documentation.
• Participate in infection prevention and quality improvement initiatives.
• Respond promptly to changes in resident condition following protocols.
• Ensure confidentiality and adherence to regulations.
