Job summary

JPS Health Network seeks a Licensed Physical Therapist for the Main campus in Fort Worth for day shifts. You will implement and adjust a plan of care for patients with impairment or disability due to injury or disease, ensuring high-quality care.

Responsibilities include assessments, planning, documentation, patient education, and collaboration with the care team. This full-time role reports to the Clinical/Prof Services - Therapy division and requires Texas licensure and relevant PT credentials.

Qualifications

• Doctorate or Master's degree in Physical Therapy required.
• Current Texas licensure required for Physical and Occupational Therapy Examiners.
• Experience may be required depending on the degree path (see above qualifications).

Responsibilities

• Performs comprehensive patient assessments and reassessments to identify functional limitations and goals.
• Develops individualized treatment plans based on physician referrals and patient needs.
• Maintains accurate documentation of care, plans, progress notes, and charges.
• Educates patients and families on diagnoses, treatment plans, home exercise programs, safety and discharge.
• Collaborates with physicians, nurses, and other team members to coordinate care.

Education

Doctorate in Physical Therapy from an accredited school or University

Current licensure by Texas Board of Physical and Occupational Therapy Examiners

Master's in Physical Therapy from an accredited school or University
