Job summary

Commonwealth of Pennsylvania through the Department of Human Services seeks a Social Worker 1 for Danville State Hospital. You will support patient care, assist with treatment planning, and coordinate with families and care teams to advance recovery goals.

You will provide counseling and help navigate community resources while addressing property and financial planning needs. You will work full-time, 37.5 hours weekly, 8:00 AM–4:00 PM with potential schedule changes.

Qualifications

• Bachelor's or Master's degree in social work or social welfare.
• Applicants will be considered to have met the educational requirements within 3 months of graduating with a qualifying degree.

Responsibilities

• Provide social work support for a unit of approximately 30 patients, including newly admitted individuals and transfers.
• Complete social histories, update assessments, participate in interdisciplinary treatment conferences, and assist with discharge planning.
• Provide individual, group, and family counseling, and serve as liaison to families and community agencies.
• Share information with staff, communicate changes in patient status, and support continuity of care.
• Assist patients with property management, arrange transportation to financial institutions, and explain financial competency rights and legal procedures.
