Qualifications

• Licensed Practical Nurse with WA license is required.
• Home health experience preferred; other settings also considered.
• Current CPR/BLS certification is required.

Responsibilities

• Provide skilled nursing visits to patients in their homes under RN direction and care plans.
• Administer medications, wound care, and treatments; monitor vital signs and progress.
• Educate patients and families on care, safety, and disease management.
• Document visits accurately and promptly.
• Communicate changes in condition to the RN case manager and physicians.
