Qualifications

• RN licensed with active status.

Responsibilities

• Provides direct patient care assessment, planning, implementation and evaluation.
• Educates patients and families on post treatment home care and medications.
• Promotes patient safety and participates in quality and regulatory activities.
• Develops and documents care plans with the interprofessional team and family.
• Maintains communication of the care plan across the continuum.
