Job summary

New Avenues to Independence, Inc. in Cleveland, OH is seeking a Day Services DSP to support individuals with disabilities in daily living, employment readiness, and community participation.

You will accompany clients to job sites, provide training, and help with personal care and daily activities in a person-centered, compassionate approach. Responsibilities include facilitating meaningful activities, maintaining HIPAA compliance, and collaborating with a professional team to implement

Qualifications

• 18 years of age or older
• Valid Ohio Driver’s license with 4 points or less, valid auto insurance
• Must have a high school diploma or GED
• A desire to make a difference in people’s lives

Responsibilities

• Accompanies individuals to job sites; observes performance; oversees work training; provides job retention training, instruction and assistance accordingly.
• Provide person-centered care and support to individuals with disabilities in a variety of settings, including residential homes and day programs.
• Assist individuals with daily living activities, such as meal preparation, personal hygiene, and medication management.
• Foster a nurturing and inclusive environment that promotes growth, independence, and self-determination.
• Plan and engage in meaningful activities, outings, and events to enhance the quality of life for the people you support.
• Promote social and emotional well-being by building strong, supportive relationships.
• Collaborate with a dedicated team of professionals to create and implement individualized care plans.
• Collaborating with people to complete tasks and build independence in their lives.
• Providing support with daily routines such as personal care, cooking, cleaning, shopping and attending medical appointments.
