Qualifications

Responsibilities

• Provide person-centered care and support to individuals with disabilities in residential homes and day programs.
• Assist individuals with daily living activities, including meal preparation, personal hygiene, and medication management.
• Foster a nurturing and inclusive environment that promotes growth, independence and self-determination.
• Plan and engage in meaningful activities, outings, and events to enhance quality of life.
• Promote social and emotional well-being by building strong, supportive relationships.
• Collaborate with a team to create and implement individualized care plans.
• Help people build independence by completing tasks and routines.
