Job summary

Midwest Family of Companies is seeking a Day Hab Specialist 2 to support individuals with intellectual and developmental disabilities in community settings. You will provide person-centered supports, help with daily living skills, and assist participants in achieving personal goals.

Ideal candidates are self-motivated, highly organized, team players with strong communication skills and a valid Ohio driver’s license.

Qualifications

• Experience in Intellectual/Developmental Disabilities field.
• Experience in Long Term Care – Assisted Living, Nursing Home, Home Health Care.
• Residential settings management experience.
• Team leadership or crew leadership in prior roles.

Responsibilities

• Provide person-centered supports to individuals with IDD in community-based settings.
• Assist with daily living skills during activities such as outings to zoos, concerts, and local trips.
• Collaborate with team to support goals and ensure safety and well-being of participants.
