Job summary

Sweetwater Care is seeking an experienced LPN with leadership skills to oversee nursing activities in a long-term care setting. The role requires administering medications and treatments, coordinating patient care, and collaborating with physicians and other staff to meet patient needs.

The ideal candidate has in-state LPN licensure, prior nursing management experience, and familiarity with long-term care operations.

Qualifications

• Currently licensed as LPN in state.
• One-year prior nursing management experience preferred.
• Long-term care experience.
• Point Click Care charting experience.

Responsibilities

• Provide nursing care within scope of practice per standards and physician orders.
• Participate in Quality Improvement Program and Infection Control.
• Utilize Life Manual for patient program placement and monitoring.
• Notify departments of patient changes and admissions/discharges.
• Conduct family and patient education.
• Administer medications timely per facility policy.
• Administer treatments timely per facility policy.
• Receive, transcribe, and carry out physician orders as allowed by law.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications
