Elite Care - Oatfield Estates in Milwaukie, OR is seeking a passionate LPN to join our care team. Provides nursing assessments, contributes to care plans, educates staff, and helps develop systems to meet state and federal regulations. Compensation ranges from $40.00 to $45.00 per hour, day shifts, full-time.

5 days/week, 8-hour shifts; weekend availability required. Qualifications include an active Oregon LPN license, 1 year LPN experience, and knowledge of Oregon Board of Nursing requirements

Qualifications

• Active LPN license in Oregon is required.
• Minimum 1 year LPN experience in a related setting.
• Strong knowledge of Oregon Board of Nursing requirements, especially Nurse Delegation.
• Ability to teach and train on Nurse Delegation.
• Knowledge of Medication Administration and Med Room procedures.

Responsibilities

• Develop and support systems for clear communication with the Director of Nursing.
• Review resident health and functional needs before move-in and as conditions change.
• Assess resident information and complete required assessments before move-in and after condition changes.
• Share on-call responsibilities on a rotational basis with qualified management.
• Ensure compliance with community policies and state/federal regulations for resident care.
• Perform select clinical tasks such as IM injections when necessary.
• Monitor residents' health status and respond to changing needs.
• Coordinate resident transfers to/from hospitals or other facilities.

Job Overview:

As a Licensed Practical Nurse with us, you'll be responsible for promoting the health and well-being of each resident by providing accurate nursing assessments, giving appropriate input to the development of care plans, and educating staff on the appropriate and proper care of each resident. You will also help to develop systems, procedures, and training programs that support governing state and federal regulations.

Compensation Range:

$40.00 to $45.00 per hour (based on experience)

Shifts:

5 days a week, , 8 hour shift. Shift days are flexible, but weekend availability is required.

Job Responsibilities (not limited to):

• Develop and support a system that allows for a constant flow of communication to the Director of Nursing regarding resident, family, or employee concerns, a decline in resident condition, employee injuries, budget disparities, or any matters that potentially impact operations
• Review and perform evaluations of resident health and functional care needs before moving in to ensure that the community can meet the resident’s needs and that regulatory requirements have been met
• Review all resident information and complete an assessment of the resident before move-in, when changes in the resident's condition occur, and as required by company policy and state regulation
• Assume on-call responsibilities on a rotational basis and assign on-call responsibilities to other qualified management
• Develop knowledge of community policies, procedures, and state/federal regulations pertaining to resident care
• Perform selected clinical tasks that reflect an unusual or unique condition in a resident or that cannot be delegated, i.e. IM injection
• Monitor the status of resident’s health condition and initiate actions in response to changing needs
• Assist in coordinating resident transfers, to or from a hospital or another health facility

Qualifications:

• 18 years or older
• Able to pass an Oregon background check
• Minimum 1 year LPN experience in related field/setting required
• Possess and maintain strong knowledge of Oregon Board of Nursing requirements, especially as they pertain to Nursing Delegation
• Possess and maintain a strong ability to teach and train on all components of Nurse Delegation
• Possess a thorough knowledge of Med Room components and Medication Administration
• Must possess an active Oregon-endorsed LPN license
• Reliable Transportation
