Job summary

Opportunities for Inclusion Inc. in Massachusetts is seeking a DayHabilitation Case Manager to support individuals with developmental disabilities.

The role requires a Bachelor’s degree in psychology or related field (or a diploma with 2+ years relevant experience), strong communication, and the ability to work with families

Qualifications

  • Bachelor’s Degree in Psychology, Special Education, or a related field required.
  • Alternate: High School Diploma plus 2+ years in direct care or case management.
  • Must obtain training: Positive Behavioral Supports, CPR/First Aid, Human Rights training.

Responsibilities

  • Manage an assigned caseload and oversee DHSP goals according to state standards.
  • Lead direct support staff and coordinate with interdisciplinary teams and families.
  • Assist participants in ADL and independence while ensuring safety and dignity.
  • Document progress with monthly/quarterly notes and maintain records for accreditation.

Position Overview

We are seeking a compassionate, organized, and proactive DayHabilitation Case Manager to join our dynamic team!

In this role, you will play a vital part in assisting participants with skill development and growth in independence across key areas, including self-help, communication, sensorimotor skills, wellness, and behavior development. You will oversee individualized Day Habilitation Service Plans (DHSP), manage an assigned caseload, lead direct support staff, and coordinate with interdisciplinary teams and families.

Key Responsibilities

  • CaseManagement & Plan Development: Manage an assigned caseload; assess participant needs; develop, write, and oversee individualized DHSP and goals in compliance with state.
  • Participant Empowerment & Care: Assist and teach Activities of Daily Living (ADL) to promote independence while maintaining participant dignity, safety, and personal choice.
  • GoalTracking & Documentation: Track participant progress, analyze data, complete monthly/quarterly notes, maintain updated records, and ensure accreditation and licensing standards are met.
  • TeamLeadership & Collaboration: Lead and direct support staff in executing daily objectives, organize room-specific meetings, participate in interdisciplinary team meetings, and integrate therapy consultant recommendations.
  • Community & Activity Planning: Design weekly schedules, plan and lead engaging community-based activities, and facilitate recreation referrals.

Qualifications

  • Education & Experience:
  • Bachelor’s Degree in Psychology, Special Education, or a related field.
  • In lieu of a degree: High School Diploma or GED plus 2+ years of paid experience in a direct care or case management role working with individuals with developmental or intellectual disabilities.
  • Certifications & Skills:
  • Strong written and oral communication skills in English.
  • Ability to obtain and maintain Positive Behavioral Supports, CPR/First Aid, and Human Rights training.
  • Basic computer literacy and willingness/capacity to learn augmentative communication systems or basic sign language.
  • Licensing & Driving:
  • Valid driver’s license required.
  • Ability to drive agency vans; access to a personal vehicle preferred.
  • Physical & Operational Requirements:
  • Physical capability to lift, transfer, pivot, and bear participant weight safely (including one- and two-person transfers).
  • Ability to work in a fast-paced, physically and emotionally demanding environment.
  • Schedule: 40 hours per week, with additional flexible hours as needed and approved.
  • Environment: Approximately 90% indoors (facility-based) and up to 30% in local community settings.
  • Meal Breaks: 30-minute duty-free meal break provided for shifts over 6 hours.
