Job summary

SCO Family of Service is seeking staff to support day habilitation programs at our Islip-area site. You will help create schedules for on-site, in-house, and community outings that meet individual and program goals, and assist with daily operations including attendance, routes, and supplies management.

You will also help with documentation, medication assistance, meals, and transportation tasks while collaborating with the Day Habilitation supervisor to plan events and life plan meetings.

Qualifications

• Associate’s degree in human services or related field.
• Minimum of two years’ experience in the ID/DD or related human services background.
• Knowledge of OPWDD HCBS Waiver Services.
• Excellent verbal and written communication skills.
• Valid driver’s license required.
• Must be able to sit, stand, and ambulate for extended periods of time.

Responsibilities

• Assists in creating a schedule and calendar of on-site, in-house, and community outings aligned with goals.
• Supports day-to-day operations, attendance, routes, goals documentation, and supplies management.
• Maintains paperwork in SCO/OPWDD standards.
• Collaborates with Day Habilitation supervisor to plan events.
• Assists with medication administration, toileting, and lunch preparation on-site.
• Attends life plan meetings and conducts staff supervisions.
• Helps develop and implement staff action plans and keeps records updated.
• Fosters positive relationships with coworkers, participants, families, and partner sites.
• Adheres to regulations and SCO policies, including incident reporting.
• May be assigned to other SCO day hab sites as needed and trains new staff.

Education
