Job summary

Vinfen's Day Hab Program seeks an Activity Assistant (Instructor) to support clinical and individual programs, implement daily plans, and assist with case management under the Day Hab Services Program (DHSP).

You will provide instruction across self-help, sensorimotor, communication, social development, and independent living domains; maintain documentation; and collaborate with an interdisciplinary team to promote meaningful engagement and community integration for persons served.

Qualifications

• One year of relevant work experience/training.
• A high school diploma or equivalent is required.
• Driving is a requirement with valid license and insurance.
• Must be able to lift at least 25 pounds and perform physical tasks.
• CPR, First Aid, NET, Safety Care trainings within specified timelines.

Responsibilities

• Provides instruction and skill building in Self Help, Sensorimotor, Communication, Social Development, Independent living, Behavior Development, Affective Development, and Wellness Development.
• Assists in case management responsibilities, as directed.
• Provides input for interdisciplinary team reviews.
• Assists in the development of DHSP for persons served, including recommendations from professionals and staff.
• Implements clinical and daily plans for persons served; maintains progress notes and documentation.
