Job summary

Qualifications

• Master's degree in Public Health, Health Sciences or related field required; terminology coursework preferred.
• Bachelor's degree required; experience in evaluation, data collection, and report preparation.
• Three years of data collection, databases, and reporting experience recommended.

Responsibilities

• Design assessment tools and surveys to build qualitative and quantitative data.
• Capture data, perform basic analyses, and drive quality improvement.
• Plan and maintain monitoring and evaluation systems and data collection tools.
• Develop annual evaluation reports with narratives and visuals.
• Ensure timely reporting of patient data to tracking systems.
• Assist navigators and providers to ensure timely follow-up and payments.
