Job summary

West Coast Wound is seeking a skilled Physical Therapist to provide home-based services in the Dallas, TX region. The PT will travel to patients' homes, perform comprehensive evaluations, and develop individualized plans of care.

Qualifications

Licensed Texas PT in good standing.
Graduate of an accredited Physical Therapy program.
Strong knowledge of anatomy, physiology, kinesiology, biomechanics.
Strong clinical reasoning and patient assessment skills.
Ability to independently perform comprehensive PT evaluations and develop plans of care.
Experience with orthopedic, musculoskeletal, neurological, balance, gait and functional limitations.
Excellent communication and documentation skills.

Responsibilities

Travel to patients' homes throughout Dallas region to provide physical therapy services.
Perform comprehensive home-based PT evaluations.
Develop individualized plans of care based on evaluation findings and goals.
Provide interventions including therapeutic exercises, gait training, balance training, and functional strengthening.
Educate patients and caregivers on diagnosis, exercises, safety, fall prevention, and independence.
Document evaluations, treatments, progress notes, reassessments, and discharge summaries.
Coordinate care with physicians, nurses, therapists, and other professionals.
Maintain compliance with regulations, payer requirements, and company policies.
