Job Summary
The Customer Service Representative (CSR)/Data Tracker serves as the primary point of contact for participants in CentroMed’s Chronic Disease Prevention Program. The CSR welcomes patients, ensures positive experiences, handles registration and processing of patients/families, coordinates encounter data entry, and manages billing, collections, and financial reviews for the program’s UDS reporting and compliance activities.
Duties and Responsibilities
- Establish and update patient accounts in the computer system.
- Enter charges for self‑pay, insurance, and eligibility programs and reconcile at day’s end.
- Determine patient eligibility for special programs such as Methodist Healthcare Ministries.
- Assist patients and visitors, greet callers, and answer telephones.
- Use Medicaid, Medicare, CHIP, and other eligibility verification tools (TMHP, Availity).
- Schedule patient appointments per established protocols.
- Maintain 100% productivity rate, complete encounter entries, logs, referrals, and outreach paperwork.
- Perform intake interview on new patients and verify homelessness status.
- Assemble medical forms for specific patient categories and initiate encounter forms.
- Follow up on no‑shows as directed by Director or Registered Dietitian.
- Maintain professional attire in accordance with policy.
- Demonstrate excellent attendance and punctuality.
- Assist clinical teams with assessments and emergency notifications.
- Follow all safety and compliance policies including OSHA and JCAHO standards.
- Participate in quality improvement activities.
- Help with outreach activities and other duties assigned by Director or Supervisor.
Requirements
- Education: High School Diploma or equivalent.
- Experience: General experience in a health care setting; preference for medical data entry experience.
- Strong clerical skills, including keyboarding and basic math.
- Ability to conduct interviews, reconcile encounter reports, and perform audits with attention to detail.
- Bilingual English/Spanish preferred.
