Allaire Rehab & Nursing in Freehold, NJ is seeking a compassionate Certified Social Worker to join our Social Services Department. This CSW role focuses on resident-centered support, discharge planning, and coordinating with families and healthcare providers to address psychosocial needs.

You’ll conduct assessments, assist with benefits and paperwork, and help residents access community resources. A NJ CSW credential and a BS in Social Work are preferred, with opportunities for professional

Qualifications

Excellent written, verbal, and resident-centered communication.
HIPAA and secure handling of medical records experience.
Familiarity with PCC and healthcare systems.
Ability to manage multiple priorities in a fast-paced setting.

Responsibilities

Conduct psychosocial, cognitive, and mood assessments, including BIMS and PHQ-9.
Assist residents with paperwork, benefits, insurance forms, and personal requests.
Research community resources, coordinate housing applications, and facilitate referrals.
Coordinate discharge planning and transitions back into the community.
Participate in interdisciplinary care planning and conferences.
Collaborate with residents, families, healthcare providers, and community agencies to coordinate care and resolve concerns.
Provide emotional support, advocacy, and assistance navigating complex psychosocial needs.
Maintain accurate, timely, and confidential documentation.
