Job summary

Allaire Rehab & Nursing in Freehold, NJ is seeking a compassionate Certified Social Worker (CSW) to join our Social Services Department. This fast-paced role supports residents with psychosocial needs, benefits, housing, and discharge planning. Experience in healthcare and familiarity with PCC are preferred.

The CSW will collaborate with families, providers, and community agencies, ensuring resident-centered advocacy and compliant documentation across electronic systems.

Qualifications

• Bachelor's degree in Social Work and active NJ CSW credential preferred.
• Healthcare/skilled nursing experience preferred.
• Familiarity with PCC and electronic health records.

Responsibilities

• Conduct psychosocial assessments and manage resident support plans.
• Assist residents with paperwork, benefits, and insurance forms.
• Coordinate housing, referrals, discharge planning, and transitions.
• Participate in interdisciplinary care planning and conferences.
• Maintain confidential documentation with HIPAA compliance.

About the Role

We are seeking a compassionate, motivated, and resourceful Certified Social Worker (CSW) to join our Social Services Department. This is a fast-paced, dynamic environment where no two days are the same. The ideal candidate is an independent problem-solver, eager learner, and adaptable professional who can manage competing priorities while providing empathetic, resident-centered support.

Key Responsibilities

• Conduct psychosocial, cognitive, and mood assessments, including BIMS and PHQ-9, as assigned.
• Assist residents with paperwork, benefits, insurance forms, and personal requests.
• Research community resources, coordinate housing applications, and facilitate referrals.
• Coordinate discharge planning and transitions back into the community.
• Participate in interdisciplinary care planning and conferences.
• Collaborate with residents, families, healthcare providers, and community agencies to coordinate care and resolve concerns.
• Provide emotional support, advocacy, and assistance navigating complex psychosocial needs.
• Maintain accurate, timely, and confidential documentation.

Essential Skills

• Excellent professional, written, verbal, and resident-centered communication skills.
• Proficiency in Microsoft Outlook, Teams, Word, and calendar management.
• Strong organization, time management, and attention to detail.
• Independent problem-solving, critical thinking, and initiative.
• Understanding of HIPAA, confidentiality, and secure handling of medical records.
• Familiarity with healthcare systems, Medicare, Medicaid, and insurance processes.
• Ability to complete assessments and maintain electronic health records.
• Empathy, cultural sensitivity, adaptability, and professional judgment.

Qualifications

Preferred:

• Bachelor's degree in Social Work (BSW) and active NJ Certified Social Worker (CSW) credential.
• Experience in healthcare, skilled nursing, case management, or human services.
• Familiarity with PointClickCare (PCC).
