• Research community resources, coordinate housing applications, and facilitate referrals.
• Coordinate discharge planning and transitions back into the community.
• Participate in interdisciplinary care planning and conferences.
• Collaborate with residents, families, healthcare providers, and community agencies to coordinate care and resolve concerns.
• Provide emotional support, advocacy, and assistance navigating complex psychosocial needs.
• Maintain accurate, timely, and confidential documentation.

About The Role

We are seeking a compassionate, motivated, and resourceful Certified Social Worker (CSW) to join our Social Services Department. Qualifications

• Preferred: Bachelor's degree in Social Work (BSW) and active NJ Certified Social Worker (CSW) credential.
• Experience in healthcare, skilled nursing, case management, or human services.
• Familiarity with PointClickCare (PCC).
