Responsibilities

• Maintain an active caseload for START services and coordinate care across systems.
• Conduct assessments, crisis planning, and cross-system collaboration.
• Provide training and consultation to teams; ensure timely data entry and reporting.
• Develop and maintain comprehensive service evaluations and intake assessments.
• Coordinate education and outreach with families, providers, and community partners.
