• Demonstrates clinical decision-making and adherence to standards of care.

Responsibilities

• Collaborate with interdisciplinary team to coordinate care and ensure safe patient transitions.
• Educate patients and families; document care and ensure understanding of discharge plans.
• Delegate and prioritize tasks to maintain high quality patient care.
• Respond effectively to emergencies and maintain required documentation.
