Qualifications

• RN license in Virginia required.
• ACLS within 90 days of hire; PALS within 120 days.
• CPR/BLS certification required.

Responsibilities

• Assess patient clinical and psychosocial needs and develop a care plan.
• Collaborate with interdisciplinary team to coordinate care and transitions.
• Educate patients and families, coordinate discharge planning and documentation.
• Delegate and prioritize tasks to support safe, effective patient care.
• Document care and outcomes in accordance with RHS standards.
